Players will need to have a current Medical Release Form for each season. Parents, this form gives your child's coach permission to seek medical care for your child if they are injured at a practice or a game that you are unable to attend. Please be sure to print a copy, complete, and give it to your coach when the season begins.

At last check, notarization is not required in Nebraska but if your child's team will be participating in tournaments please check each tournament's rules to be sure notarization is not required.

Please give the medical release form directly to your child's coach or team manager!
